When you're ready to leave the hospital, you can take part in our discharge medicines service. This service puts you directly in touch with our branch pharmacist. We can then talk about your medicines directly with you.

Our pharmacist will take the time to review your discharge medication, talk you through pre-hospital medication and any changes made. We will ensure you are aware of why medicines have been started or stopped and how best to manage taking them as prescribed. We will talk you through the re-ordering of your repeat medicines to ensure you never go without. By understanding your medicines and how best to take them, you help ensure you receive the maximum benefit from them.
Our pharmacist will support you to feel confident in managing your medicines at home and will put additional assistance in place if require, helping you maintain your independence. This service is entirely free of charge and our pharmacy team will work closely with your GP Practice in order to assist you.
How this service can help you
About half of all medicines are not taken as they are prescribed. Common errors include:
- Taking the wrong dose (amount)
- Taking the medicine at the wrong time
- Not taking the medicine at all
If you do not take your medicines as prescribed, your condition is not treated correctly and could even get worse. By understanding your medicines and the best way to take them, you can make sure that they help you as much as possible. Our pharmacist will support you to feel confident about managing your medicines at home. This is a free service and our pharmacist works closely with your GP.
